I certify that I am eligible for the fee reduction claimed in this application. Further, I understand that this claim for reduction is subject to audit. All applications are reviewed and approved by the Office of Benefits administration.

I understand that fees reduced for courses taken at the graduate level will be added to my W-2 taxable earnings. Further, I understand that the University will withhold the appropriate taxes from my pay for fees at the graduate level.

When graduate level courses are dropped, the amount not refunded in accordance with University Refund Policies will be added to my W-2 form and subject to taxation.

In cases where the reduction entitlement for a dependent is based on a former employee who is deceased, and whose spouse is also deceased, the tax liability on the reduction will be the responsibility of the student.

Details of the fee reduction benefit can be found online in the Board of Trustees University Rules . Applicable rules are 3359-20-04.3 and 3359-26.02.
